Solution

Here’s how to construct an ODT template from scratch to generate a form with Carbone.

First, manually design the form (use tables for better alignment), then insert text boxes and checkboxes.

Notes:

• To remove the borders of the text boxes: right-click on the text box, select Control Properties from the submenu, go to the General tab, scroll to Border, select Without frame.
• Don't copy and paste checkboxes in the template unless you assign a different Label to each one.
